Introduction: In a remote, hospital-at-home program, patients receive medication blister packs in a bag from the local pharmacy with no way to organize them. This may cause confusion for the patient when determining what medications are due. The purpose of this project is to determine if the implementation of a medication organizer will decrease the rate of medication errors in the home hospital program by 5%.Methods: This quality improvement (QI) project was conducted in a large healthcare system's hospital-at-home program over three months. This was a pre- and post-intervention model. A total of 261 participants were enrolled into the program during the post-intervention period.Results: 53.3% of participants were male (n = 139) and 46.7% were female (n = 122). The mean age was 69 (SD = 14.94). The mean number of medications prescribed to a patient was 17.8 (SD = 6.99). Borderline significance (p = 0.0547) was found between the number of medications prescribed and the likelihood of having a medication error.The testing of incidence rates between the pre- and post-intervention periods weredetermined as borderline significance (p = 0.0566). The results demonstrate that the number of medication errors was reduced from 32 medication errors pre-intervention to 17 medication errors post-intervention for a reduction rate of 43.2%.Conclusions: Medication errors are prevalent in many healthcare settings and can have detrimental impacts on patient safety. While the intervention of this QI project was not significant, the results were favorable and reduced the number of medication errors by 43.2%.
