The aging process is characterized by gradual decline in cognitive functioning; however, the extent of decline can vary depending on health factors. Older adults are more susceptible than the general population to developing metabolic syndrome (i.e., three or more cardiovascular risk factors; MetS) and subsequent physical limitations. Furthermore, this population is also at higher risk of experiencing traumatic brain injury (TBI) which can exacerbate cognitive and mobility decline. Recent research has demonstrated that executive functioning and explicit memory ability have the potential to moderate the negative association between MetS and mobility in older adults (Ogawa et al., 2020). The current study sought to build upon such research by testing a mediation model on the role of cognition in the association between MetS and mobility decline in older adults with and without histories of TBI. All cross-sectional data for this study were gathered from the National Alzheimer's Coordinating Center Uniform Dataset. The study first used an independent samples t-test to compare mobility performance between the participants with and without histories of TBI. Second, using four independent samples t-tests, the participants with and without histories of TBI were compared on individual measures of processing speed, executive functioning, immediate recall, and delayed recall. Lastly, using two separate parallel mediation models for participants with and without histories of TBI, this study tested the strength of any indirect effect of MetS on mobility performance through each cognitive variable. Results indicated that participants with a history of TBI performed significantly worse on a measure of mobility limitation (p <.001); these results were still significant after controlling for age and education (p <.001). Participants with a history of TBI performed significantly better on the measures of processing speed (p = .001) and executive functioning (p = .028) while no significant differences were found in immediate or delayed recall performance; these results were not significant after controlling for age and education. The mediation analyses revealed that MetS had a significant indirect effect on mobility through processing speed (b = .1758, 95% CI [.0805, .2812]) and executive functioning (b = .0479, 95% CI [.0010, .1057]) abilities within the participants with history of TBI. Similarly, MetS also had a significant indirect effect on mobility through processing speed (b = .1352, 95% CI [.1037, .1680]) and executive functioning (b = .0720, 95% CI [.0559, .0894]) abilities within the participants without a history of TBI. There were no significant indirect effects for immediate or delayed recall in either group. Findings elucidate a potential pathway by which MetS contributes to mobility decline in older adults through specific reductions in processing speed and executive functioning capabilities.
