Healthcare delivery systems are perilous in nature and have the potential to cause harm with every encounter. Healthcare organizations are responsible for adopting and maintaining patient safety programs that foster a culture of safety that aids in avoiding unintended harm. However, the literature insinuated that despite the continuous efforts to shift the safety culture mindset, adverse events are still prevalent. The purpose of this research was to explore healthcare professionals' attitudes toward safety culture and answer the research question: How does the implementation of a hospital-based patient safety program influence healthcare professionals' perceptions and behaviors toward patient safety? This study adhered to a non-experimental qualitative method and case study approach to conduct a patient safety program evaluation at a single acute care hospital. Observations and survey responses of healthcare professionals were analyzed in the study. Findings included five common themes: a) resource allocation influences patient care outcomes, b) core values impact the delivery of care, c) patient safety is a universal responsibility, d) leadership is fundamental to building safety culture, and e) transparency increases psychological safety and decreases harm. This study suggested that healthcare organizations should develop a plan for continuous education, perform an annual patient safety program evaluation, seek out alternative nursing patient care models to alleviate resource deficits, and adopt a just culture model to cultivate a culture of safety grounded in accountability and transparency.
