Antimicrobial resistance (AMR) is a recognized global crisis, threatening human and environmental health. Water resources are considered the primary AMR environmental reservoir and dissemination pathway; yet, only a few studies have focused on understanding AMR in vulnerable karst groundwater systems. This is particularly true for urban karst areas, which provide drinking water for nearly two billion people. To advance the scientific knowledge of AMR in karst groundwater systems, this interdisciplinary research aimed to understand the prevalence of antimicrobial resistant bacteria (ARB) and genes (ARGs) in urban karst systems, evaluate how the unique karst environment and hydrology may influence ARGs, and assess the current global and national AMR stewardship actions and policies for the inclusion of karst groundwater resources. Water samples from ten urban karst groundwater features in Bowling Green, Kentucky collected weekly for 46 weeks, and 45 spot samples in Tampa Bay MetroArea, Florida were analyzed for AMR and a suite of water quality parameters. A combination of high-throughput and quantitative PCR methods and statistical analyses were used to to determine the occurrence of ARGs, and understand the relationship between resistance and the groundwater system. This study found that ARGs are prevalent throughout urban karst groundwater systems, irrespective of season or water quality with a significant occurrence at spring features. The ARGs are contaminants introduced through anthropogenic activity into unregulated karst systems. The aquifers, in turn, become AMR reservoirs and dissemination pathways. To protect these systems, new AMR regulations are needed to protect vital drinking water resources and human health.
