Public health initiatives in low and lower middle-income countries (LMICs) are funded from a variety of sources, including from development assistance for health (DAH). DAH involves funding and technical assistance that targets regions with suboptimal per capita health spending and economic development. DAH sources include bilateral government partnerships, private/family philanthropies, multilateral partnerships, and host governments. As of 2016, DAH accounted for 25.4% of total health spending in low-income countries and 3.2% in lower middle-income countries. Regardless of the motivation behind DAH, the percentage that DAH has contributed to overall health spending in LMICs should prompt reflection about its effectiveness. The term "vulnerability" is widespread in DAH. Given how commonly the vulnerability concept has been considered in DAH funding decisions, it is important to recognize that variations in its constructs and applications within the context of epidemiological and economic transitions could affect which geographic areas receive DAH and how effective DAH could be. This study examined how the constructs/applications of vulnerability in DAH inform more effective resource allocation decisions in LMICs. The study examined variations in the constructs and applications of the concept of vulnerability in DAH and approaches used in resource allocation to determine which constructs and applications support more effective resource allocation. Two principles of the Paris Declaration on Aid Effectiveness were used to frame the concept of DAH effectiveness in this study. Some key findings were as follows: 1) a perception that DAH provided to LMICs was DAH that already targeted vulnerable populations; 2) one universal construct may be unnecessary and unrealistic; 3) capacities to address multidimensions of vulnerability may differ by income-level; and 4) meeting needs, donors' interest, disease burden, and country's willingness/capacity were strong considerations for funding public health programs in LMICs. Attaining alignment with a country's ideas/needs appeared to be the top attribute of an effective allocation of resources. Organizations with clearer constructs did not consistently apply them to inform resource allocation and accountability measures.The study concluded that organizations that concretely described the populations they considered vulnerable had clearer throughlines to resource allocation and expected results, which improved the effectiveness of their DAH.
