Hospital readmissions and emergency department (ED) visits after discharge are associated with an increase of morbidity and mortality in patients as well as an increased cost to healthcare. Healthcare systems must evaluate readmissions and the discharge process to fill gaps in care to help reduce readmissions. This project utilized an evidence-based method to reduce readmissions by performing follow-up phone calls 48-72 hours after discharge. Plan, Do, Study, Act was the framework utilized for this project. Pre-implementation readmission rate was 12.5% in the 6 weeks prior to performing phone calls. ED visits were not tracked, and this project will serve as baseline data to trend that statistic. After project implementation the readmission rate was 8.24%, and the ED visits rate was established at 10.3%. Preventing readmissions and ED visits is important to patients and the healthcare facility in order to improve patient care and outcomes. This project found that performing post-discharge follow-up phone calls after discharge can potentially reduce readmissions and ED visits and improve the overall quality of patient care.
