Background: Scapular winging is a rare disorder that is commonly caused by nerve damage of the dorsal scapular nerve, spinal accessory nerve, or the long thoracic nerve. This affects the scapulohumeral rhythm which may cause abnormal kinetic motion of the shoulder. Purpose: The purpose of this case report is to describe an outpatient physical therapy intervention approach incorporating scapular strengthening to reduce scapular winging symptoms in a patient with a long thoracic nerve injury. Case Description: Patient was a 15-year-old male with a long thoracic nerve injury who was status post subacromial decompression. The patient experienced scapular winging, dyskinesis, and a decrease in shoulder range of motion 8 months prior to the initial evaluation. The patient’s symptoms had progressed since the mechanism of injury. The patient received physical therapy 1-2 times per week for 6 weeks with goals of scapular strengthening and protection. Result: The patient made improvements in the shoulder pain and disability index (SPADI) outcome measure. However, significant scapular winging and limited shoulder range of motion were still present. Discussion: This case report provides a plan of care framework for patients with long thoracic nerve injuries in an outpatient setting. General practitioners and physicians should consider the possibility of long thoracic nerve injury with scapular winging and restricted overhead arm movement before performing a subacromial decompression surgery.
