Rosacea is a common skin disease that causes papules, pustules, redness, visible vessels and hot-flushing feeling on the central part of the face. Side effects and rebounds always occur even after the recovery from current western medicine treatment. The objective of this study is to explore if the intake of Chinese herbal medicine named Yin Qiao San（銀翹散） can relieve the skin conditions of Rosacea and improve the quality of patient life. A total of 60 study subjects have been randomly divided into two groups, i.e. Placebo (P) and Chinese medicine (CM) groups. To qualify the efficacy of interventions, the four basic Chinese Medicine diagnostic methods, image-taking by German skin analysis machine, SF-36 questionnaire (Quality of Life questionnaire) comparing with SF-12v2 HK norm, Investigator's Global Assessment (IGA), Patient’s global assessment (GA) and Patient’s self-rating scale (from 0 to 4) of skin have been done during the 8-week study. The study has been approved by the Ethics Committee on March 30, 2017. The facial expression, skin lesion, tongue pictures and pulse conditions have been improved after the treatment. For the facial analysis change, CM group has been improved more than P group after treatment (but not significantly different). However, younger patients (age≤34) of CM group have been improved significantly in all 4 parameters than P group. For the SF-36 assessment, Vitality, social functioning, mental health and mental component summary of the study subjects were significantly worse than HK norm at baseline. QOL for physical functioning of CM group was significantly improved from baseline. CM group was also significantly less affected by high temperature after treatment. For the patient self-rating assessment, 26 subjects (86.7%) of CM group were improved after the first course of treatment. They were then improved gradually and progressively. 3 subjects (10%) rated themselves totally recovered. 14 subjects (46.7%) rated themselves recovered over 70%. Meanwhile, 60% subjects of P group felt unsatisfied with the effects (no change/getting worse) after the first course of placebo even though they rated themselves improved after the treatment for the eight consecutive weeks. None of them (0%) rated themselves totally recovered. 4 (13.33%) subjects rated themselves recovered over 70%. For Investigator's Global Assessment (IGA), Investigator score was significantly better in CM group than P group. In this study, CM group was improved more than P group after treatment (but not significantly different) in facial analysis. However, younger patients (age≤34) of CM group particularly showed significant decrease in the degree of inflammation after the treatment of Chinese medicine. P group also had improvement in different aspects. This explains a Chinese medicine practitioner does not only use herbal medicines to treat health problems of patients. Medical advices and mental support are also important during the treatment of rosacea. Although rosacea is just a skin problem, it causes low self-esteem and emotional distress. Besides just relying on herbal medicine, medical advices and encouragement are also critical during the treatment. Universal, geographical and human factors should also be taken into account. This is the holistic approach of Chinese medicine which can both help improving the skin condition and developing sense of wellbeing. This provides an important hint for the future medical treatment of rosacea.
