Background: The prevalence of childhood eczema is increasing and it currently affects approximately 15% of Hong Kong children and their families physically, psychologically and socially. A lack of education on managing eczema and limited time for specialists’ consultation in Hong Kong aggravates this problem. Evidence suggests that structured educational programmes on eczema management significantly reduced eczema exacerbation. An integrative review was conducted to identify the components of effective interventions to inform the development of educational programmes for Chinese population. Phase I: An integrative literature review was conducted and summarised the evidence from 15 randomised controlled trials (RCT) involving 2,608 participants. Eight included trials demonstrated a significant reduction in disease severity of children with eczema and suggested educational interventions targeting parents. However, there is inconclusive evidence regarding the optimal dose and lack of a conceptual framework to guide these interventions. Valid and reliable eczema outcome measures for Chinese population were also not available. Phase II: Three eczema measuring tools on parental self-efficacy, treatment adherence and quality of life (QoL) of family members were translated into Traditional Chinese. The psychometric properties of these tools were examined in 147 parents or caregivers of children with eczema recruited at a specialty outpatient clinic (SOPC) in Hong Kong. Phase III: A single-blind RCT was conducted at a SOPC of a regional hospital in Hong Kong. The aim was to evaluate the effectiveness of an innovative nurse-led parental eczema education (PEE) programme on health outcomes of children with eczema and their families. Eligible participants were randomly assigned to either an intervention or a control group. Parent participants of the intervention group received a 30-minute, face-to-face, nurse-led, individual session and were arranged to join an online group sharing forum for three months. The programme materials included a newly-designed booklet and demonstration videos on managing eczema and progressive relaxation exercise. Child participants of both groups received standard medical treatment. Outcome measures included disease severity of children with eczema, parental self-efficacy, treatment adherence and QoL of family members. Data were collected at baseline (T0) and three months after the commencement of the intervention (T1). Generalised estimating equation model was used to evaluate the between-group differences in outcomes across time. Results: A total of 136 child-parent dyads were recruited and three dyads dropped out in the study (2% attrition rate). The results showed that the participants in the intervention group, when compared with the control group, had significant improvements in disease severity of children, parental self-efficacy, treatment adherence and QoL of family members, at 3-month follow-up assessment relative to baseline. Majority of the parent participants in the intervention group commented that the PEE programme was beneficial. However, low degree of participation in the online sharing forum was found. Conclusion: The study supported that the nurse-led PEE programme was effective in improving child, parent and family outcomes. Besides, the PEE programme could guide healthcare providers in delivering eczema education and facilitate the transfer of knowledge to support parents in caring for their eczematous children.
