This qualitative research study was conducted with assigned female at birth (AFAB) Black women1 in the Mid-Atlantic region of the United States of America. The author used a grounded theory approach to conduct nine semi-structured interviews with questions inquiring about the experience of interacting with reproductive healthcare providers and the reproductive healthcare system. Questions also inquired about interviewees understanding of reproductive health and the societal conceptualization of reproductive health. A constant comparative, open-coding method of analysis was utilized, and emerging themes were noted and discussed between the two coders: the author and a research assistant.Findings from semi-structured interviews revealed three overarching themes: 1) Sources of Self-Perception; 2) The Landscape of Reproductive Health Care; and 3) Sources for Determining a More Just Care System. The findings demonstrate a picture of reproductive healthcare for Black women and the lived experience of navigating healthcare. AFAB Black women engage in meaning-making of their racial identity which ultimately structures how they approach interactions within reproductive care settings. Findings give guidance on how providers can better understand the many factors influencing how AFAB Black women are experiencing reproductive healthcare which offers vital insights on how to promote better health outcomes. Additionally, analysis produced a theory of reproductive exclusion. The proposed theory presents an ecological-type model, with a person at the center with closer and further environmental influences shaping their experience, to explain the difference of experience for AFAB Black women. A theory of reproductive exclusion gives the medical field a starting point for understanding AFAB Black women's relationships to reproductive healthcare. Social work curriculum should include these findings when training social workers on how best to support AFAB Black women receiving healthcare services. Social work education would also benefit from discussions of visual data collection methods to empower communities to tell their own stories.
