Acceptance of COVID-19 vaccine is linked to positive healthcare outcomes with minimal side effects; however, some individuals hesitate to access this vaccine. The problem addressed by this study was COVID-19 vaccine hesitancy among BSN students in the United States which can exacerbate the existing nursing shortage. Guided by the health belief model, the purpose of this quantitative correlational study was to determine the relationships among health literacy, program level, political affiliation, vaccine experiences, annual family income, ethnicity, and COVID-19 vaccine hesitancy in undergraduate BSN students. The research questions addressed whether program level, health literacy, political affiliation, vaccine experiences, annual family income, and ethnicity could predict individual COVID-19 vaccine hesitancy. Undergraduate BSN nursing students (N = 191) were recruited via SurveyMonkey Audience to respond to a survey which included previously validated Health Literacy Instrument for Adults (HELIA) and COVID-19 Vaccine Hesitancy Scale. Data were analyzed using a multiple linear regression, revealing decreased health literacy, Republican political affiliation, no listed political affiliation, and Asian ethnicity significantly predicted COVID-19 vaccine hesitancy. Recommendations for future practice include a wider range of considerations for COVID-19 vaccine exemptions, teaching health literacy to undergraduate BSN students through a structured curriculum, and targeted interventions. Future research should focus on additional predictors of COVID-19 vaccine and booster hesitancy and comparison of BSN students' COVID-19 vaccine hesitancy compared to other healthcare students. The current study expanded the utility of health belief model as it established health literacy, political affiliation, and ethnic background as modifying factors that can predict individual hesitancy surrounding the health behavior of COVID-19 vaccination. With the continued development of COVID-19 vaccines, understanding COVID-19 vaccine hesitancy is needed to ensure an equitable and effective response to the COVID-19 pandemic.
