The purpose of this dissertation was to examine the hemodynamic and neuromuscular fatigue responses to locomotor exercise in healthy individuals and patients with essential hypertension. The first study examined the role of increasing nitric oxide bioavailability, via dietary nitrate supplementation, on the peripheral hemodynamic and neuromuscular fatigue responses to cycling exercise. Briefly, these data demonstrate that despite reducing exercising leg blood flow and likely oxygen delivery, dietary nitrate supplementation does not improve exercise tolerance or change the development of neuromuscular fatigue during leg cycling.The second study sought to elucidate the role of group III/IV leg muscle afferent feedback on the central and peripheral hemodynamic response to leg cycling exercise. Pharmacological blockade of these sensory neurons significantly decreased cardiac output and blood pressure, increased leg vascular resistance, but did not affect leg blood flow and oxygen delivery during exercise. These findings indicate that group III/IV muscle afferents significantly contribute to the sympathetically-mediated increases in central hemodynamics and vascular resistance associated with locomotor exercise in healthy individuals.The third project aimed to characterize the central and peripheral hemodynamic response and neuromuscular fatigue development during locomotor exercise in patients with essential hypertension. Compared to well-matched healthy individuals, patients with hypertension displayed a similar cardiac output, but augmented blood pressure and leg vascular resistance, and a reduced leg blood flow response to cycling exercise. Additionally, the patients with hypertension were characterized by a faster rate of development of both central and peripheral fatigue during exercise. This investigation indicated that hypertension impairs the peripheral hemodynamic response to exercise and subsequently exacerbates the development of neuromuscular fatigue during locomotor muscle exercise in humans. In summary, this dissertation characterized mechanisms determining the hemodynamic and neuromuscular fatigue response to locomotor exercise in health and disease.
