In patients with prostate cancer (PCa), the radical prostatectomy (RP) is a morbid surgery with vast implications for quality of life (QoL), as patients suffer physical and psychological symptoms for years postoperatively. Little is known about the psychological associated factors that influence long-term QoL. One proposed moderator is patient self-efficacy for healthful behaviors. The objective of this study is to assess the relationship between patient-reported self-efficacy for nutrition and exercise and long-term QoL in men with PCa post-RP. Our study sample (n=31) included participants of the WARRIOR trial between 3-5 years post-RP (mean=4.67 years). We administered a questionnaire that assessed self-efficacy for nutrition, self-efficacy for exercise, and long-term QoL. Linear regression modeling was performed between self-efficacy and QoL scores. Models considering percentage of weight lost during the trial and stratification for social vulnerability were analyzed as exploratory outcomes (α= 0.05). Linear regression modeling showed that self-efficacy for nutrition and self-efficacy for exercise can account for a significant amount of variability in long-term QoL. The models show that self-efficacy for nutrition can increase QoL by a factor of 0.89 (95% CL 0.56-1.2, p= 0.008) and self-efficacy for exercise by a factor of 0.61 (95% CI 0.41-0.80, p=0.005). Our data show that a significant relationship may exist between one's confidence in their ability to continue a healthy lifestyle and their QoL post-RP. Clinicians should implement motivational interviewing to maintain patients' confidence in continuing healthful behaviors in the extended postoperative period.
