The absence of an evidence-based guideline-directed supervised exercise therapy protocol partnered with no operating vascular wellness program for individuals with symptomatic peripheral artery disease in eastern North Carolina created variations in clinical practice. These variations impact the quality of care delivery and outcomes of individuals diagnosed with peripheral artery disease. The main purpose of this Doctor of Nursing Practice project was to develop and recommend adopting an evidence-based supervised exercise therapy (SET) protocol into current treatment practices of individuals with symptomatic peripheral artery disease at a private cardiology group practice in eastern North Carolina. The Donabedian model of quality improvement was utilized to frame the translation of current evidence into best practice, while Kotter's eight-step change model informed the implementation process associated with the organizational adoption of the novel evidence-based SET protocol. Expert cardiology practitioners (n=5) utilized the Appraisal of Guidelines for Research & Evaluation II (AGREE II) as an assessment tool to determine the quality and applicability of the developed SET protocol. The evidence-based SET protocol overall assessment achieved a score of 100 % leading to five expert appraisers recommending the SET protocol for adoption into the current practice of treating individuals with symptomatic PAD. Due to the 100% agreement, a chi-square test was not performed. None of the providers rated disagreement. This overall finding was clinically significant as all providers supported and agreed with the guidelines for implementing the SET at the clinical site.
