Background: Chatbots could be a scalable solution to the lack of mental health resources. Chatbots have shown promising results, however, there is limited information about how to develop them, assessment best-practices, and how people utilize them. Study I aimed to understand how a chatbot for adult depression is utilized. Study II aimed to understand the efficacy of assessment through a chatbot. Study III aimed to understand how adult users perceive a chatbot. Study IV explored adolescents' perceptions of a depression chatbot. Results: In Study I it was found that there was large heterogeneity in user engagement across different modules, which appeared to be affected by the length, complexity, content, and style of questions within the modules and the routing between modules. In study II it was found that 99.82% of participants completed the PHQ-9 successfully and that it had strong internal consistency. A one-factor structure was found to have good model fit. The results of study III showed the overall average net promoter score was 8.67. The following five user themes were found across the qualitative responses: positive outcome, users did not address the question, bonding with the chatbot, negative technical aspects, and ambiguous outcome. In Study IV it was found that the average NPS score was 6.04 (SD = 2.18), and 64.3% (n = 9) said they would use the chatbot in the future. The most common impressions were related to symptom improvement (61.1%) and availability (52.8%). Users recommended solving technical problems. Discussion: Study I showed that participants engaged with the chatbot; however, there was a heterogeneous usage pattern because of varying module designs. Study II showed there is initial support to the utilization of the PHQ-9, and similar assessments, via chatbot. Study III showed that a chatbot for social isolation and loneliness was perceived positively by most participants that provided feedback. A minor portion of the users reported dissatisfaction with chatting with a machine. Study IV showed that chatbots for depression are acceptable to adolescents. Most participants reported positive experiences with the chatbot. Adolescents highlighted technical and stylistic problems that developers should consider.
