Total knee arthroplasty (TKA) enhances functional outcomes, reduces pain, restores knee joint alignment, and improves overall quality of life compared to end-stage OA. However, participants post-TKA still fail to meet healthy age matched functional outcome levels due to observed kinematic gait deficiencies. Post-TKA population report higher risk of falls compared to healthy adults, and deficient balance is believed to be the cause of the reported gait abnormalities. Yet, predictors of balance deficiencies are unknown, although it was previously suggested that reduced muscle strength and knee proprioception may play a role in increased risk of falls. To date, contradictory results have been reported regarding the effect of TKA on knee joint proprioception, in addition to using balance as a surrogate for single joint proprioception measurement. Moreover, there are limited published research studies investigating the effect of knee proprioception on static and dynamic balance deficits post-TKA. Accordingly, this thesis first aimed to investigate knee proprioception in participants after TKA, and assess whether multi-joint balance is an appropriate measure of single joint proprioception post-TKA. The second aim focused on measuring the effect of knee proprioception on static and dynamic balance. Our main finding indicates that participants one year after primary unilateral TKA with cruciate ligaments scarified and collateral ligaments retained show deficient knee proprioception. We also report clinical balance assessments as an inappropriate measure of single joint proprioception. Our findings mainly report participants after unilateral TKA with knee proprioception deficits show more rigid and less healthy static balance pattern compared to participants with less deficient knee proprioception. Finally, we report TKA group being at a higher risk of falls when landing on the surgical limb during self-selected over-ground walking. However, the underlying causes are still unknown. Further research is still needed to investigate the underlying causes to a deficient knee proprioception post-TKA to implement the proper rehabilitative program.
