Young adults with intellectual and developmental disabilities and mental illnesses face an array of challenges that marginalize them from the general population. To become a healthier society, we must address these issues, including the need for long-term care and housing. The current system for identifying an appropriate long-term care home is ineffective and contributes to delayed placements. The consequences of which are negative outcomes such as homelessness, unnecessarily long hospital stays, family and caregiver stress, and monetary costs to these systems and the government. The benefits of well-matched care and housing include improved quality of life, better health, and less of the above-mentioned costs. Using an individual's skills and care needs is a way to identify appropriate housing for them and it can be done efficiently with the use of the internet.This project is a white paper that argues for a change in practice and provides a recommendation for an internet-based resource that uses current evidence to best pair adults with intellectual and developmental disabilities and mental illnesses with adult family homes; "Be My Neighbor." While helping save costs associated with the lack of placement, it also achieves local and national health initiatives. Recommendations for a pilot study to evaluate the effectiveness of intervention are also made.
