Background: Research combining inertial and muscle activation measurements of the upper extremities in older subjects is scarce, and conflicting information exists on how pain impacts thoracohumeral biomechanics. The purpose of this dissertation was to expand the knowledge of shoulder complex active range of motion (AROM) using inertial measurements, and to explore differences in shoulder complex motion or muscle activity based on subject age or pain.Methods: Asymptomatic and symptomatic subjects (aged 20-45), and asymptomatic subjects (aged 65-85) were recruited (N=55; asymptomatic younger: 22; asymptomatic older: 22; symptomatic younger: 11). Inertial sensors were placed on occiput, T1, T12, and bilateral upper arms, wrists, and hands. Surface electromyographic muscle activity from the deltoid, trapezius, and serratus anterior was recorded. Subjects performed flexion, abduction, overhead shelf, cross-body, and ipsilateral reaching activities. Joint kinematics, muscle activity, and scapular muscle activation ratios were determined and compared between groups. Findings: Asymptomatic younger subjects exhibited significantly greater thoracohumeral flexion (p=0.005) and abduction (p=0.004) during flexion and abduction activities respectively, and greater thoracohumeral internal rotation (p=0.006) during cross-body reach compared to older subjects. The older group showed significantly greater scapular upward rotation (p<0.001) during cross-body reach, larger mean activation for glenohumeral and scapulothoracic muscles (p≤0.007), and greater upper trapezius/serratus anterior (p≤0.001), and lower trapezius/serratus anterior ratios (p<0.001) versus the asymptomatic younger group. No significant between-group differences for thoracohumeral or scapulothoracic ROM were found based on presence of shoulder pain (p-value range: 0.060 - 0.903). Symptomatic subjects exhibited significantly greater anterior (p=0.001) and middle deltoid (p=0.002) activation values during the shelf reach activity versus asymptomatic subjects.Interpretation: Older adults exhibited decreased thoracohumeral AROM during maximal elevation of the dominant arm. Clinicians should consider patient age when determining rehabilitation goals to avoid over or underestimating rehabilitation potential. Additionally, older subjects exhibited significantly greater scapular activation ratios (trapezius/serratus anterior) compared to younger subjects. This observation mirrors findings in subacromial impingement syndrome, so scapular retraining exercises might be used as a preventative measure in older individuals.
