Anxiety disorders are the most prevalent mental health disorders of childhood and adolescence worldwide and are one of the top 10 contributors to the global burden of disease for youth (Kyu et al., 2016). Childhood anxiety disorders may have a significant negative impact on educational, social, occupational, familial, health, and mental health functioning, extending from childhood into adulthood. Despite the growing knowledge on anxiety disorders in youth, these disorders tend to be under-recognized and misdiagnosed in children (Lee, 2018). For those that do receive intervention, there is a subset of youth that continue to experience symptoms of anxiety following treatment with CBT (e.g., Ginsburg et al., 2014). As such, early identification, prevention, and more targeted interventions are crucial for effectively treating pediatric anxiety disorders to avoid acute and long-term negative outcomes into adulthood. One of the ways to bolster early identification and preventative intervention is through the examination of risk and protective factors associated with their development and trajectory in a developmental psychopathology and/or multi-systemic perspective (e.g., see Davies & Cicchetti, 2004; also see Masten et al., 2021). As such, the purpose of this systematic review was to explore risk and protective factors for adolescent anxiety disorders, and in turn, potentially inform prevention programs and targeted interventions in this population. Following PRISMA guidelines, an exhaustive search of PsycINFO, PubMed, and Ebsco Academic Search Elite identified 7413 documents. After duplicates were removed 5327 articles were screened and 5102 articles were excluded. The review of the included studies (N = 28) included 460,759 adolescents across all reviewed studies aged 9.6 to 19.1 (Mage = 14.4) and identified the following risk factors: neuropsychological/neurobiological, temperament/personality traits, trauma, prenatal factors, prenatal mental health, parental factors, parental mental health, pubertal development, sleep disturbances, physical health conditions, gender, and comorbidity. The most common protective factors were maternal and paternal factors and interpersonal support/social connection. Thematic analysis of these studies resulted in themes of 'Preventative care,' with three subthemes, including 'Universal prevention,' 'Selective prevention,' and 'Indicated prevention.' Each subtheme included the most appropriate and effective targeted interventions within each category. With the information gathered in this review, clinicians and researchers must continue to create prevention programs and specifically address the risk and protective factor identified in this review, and continue to investigate their efficacy as well as any barriers to their implementation.
