Background: Gaining insight is a central mechanism of therapeutic change and is considered a cornerstone and an integral component of psychodynamic psychotherapy. Although insight has different meanings in different psychotherapy traditions, it has been suggested that it is a common factor that contributes to success in different types of psychotherapy orientations Insight may be defined as a conscious event of a shift in meaning that patients achieve by making associations between aspects of past and present experiences, typical relationship patterns, and the relations among interpersonal challenges, emotional experience, and psychological symptoms. Theoretically, insight is considered to be an indicator of better mental health, and it is expected to be related to therapeutic process and outcome.A recently published meta-analysis that systematically examined the association between insight and psychotherapy outcome found a moderate-sized effect. However, there was significant heterogeneity across studies, which can potentially he explained by several factors, including the following: the diversity of methods for assessing insight and by investigating insight as one construct, without distinction between two aspects, pretreatment individual differences, and changes in insight during treatment. The overarching goal of the present thesis was to deepen our understanding of insight while considering insight as a more complex, objective, and hierarchical structure, and investigating the role of insight in the process and outcome of psychotherapy.Aims: To accomplish the overarching goal of this dissertation, we had three aims. The first aim (Study 1) was to develop a comprehensive coding system for the Self-Understanding of Interpersonal Patterns Scales Interview (SUIP-1; Gibbons & Crits-Christoph, 2017), which will overcome the biases of the available measures of insight and consider insight as a more complex structure. The second aim (Study 2) was to examine the relation between insight and the therapeutic process (alliance). We investigate whether patients with different levels of insight at pretreatment will benefit from distinct techniques (supportive vs. expressive) in improving alliance. The third aim (Study 3) was to examine the relation between insight and outcome. We examine the association between patients" insight and psycho pathological symptoms by investigating the dual perspective (convergence between the perspectives of patient and professional evaluator) on insight and disentangling two potentially distinct components of insaght: pretreatment individual differences, and changes in insight during treatment.Method:For Study I and Study 2, data was obtained from patients enrolled in an ongoing randomized control tnal of supportive-expressive psychodynamic psychotherapy for depression. All patients provided informed consent. Insight was assessed using a semi-structured interview (SUIP-I; Gibbons & Crits-Christoph, 2017) based on the coding system we developed in Study 1. For Study 1. To examine the convergent and discriminant validity, self-report measures were used (alliance expectations, Interpersonal affiliation and avoidance attachment for convergent validity and self-esteem for discriminant validity). For Study 2, the psychotherapy process (therapeutic alliance), was assessed using a self-report measure. Study 3 included patients who sought treatment at the university counseling center. All patients provided informed consent. Insight was assessed by a self-report question (for the patient's perspective and the professional evaluator's perspective). Psychotherapy outcome (psychopathological symptoms) was assessed using a self-report measure.
