Background: The number of older Americans is projected to make up 23% of the total population by 2050. Since aging is an independent risk factor for acquiring cardiopulmonary diseases, this large demographic shift poses significant implications with regards to the management of such diseases. Gait velocity has been identified as the sixth vital sign in older adults and is a strong predictor of functional decline. Therefore, the purpose of this research study is to measure self-selected gait velocity in older adults who were admitted to an inpatient rehabilitation facility with a cardiovascular or pulmonary primary diagnosis.Methods: This observational and correlational study was conducted from 11/30/2022 to 5/1/2023. Participants consisted of 14 older adults with a mean age of 79.5 years (SD 10.54). Participants were recruited via convenience sampling from San Joaquin Valley Rehabilitation Hospital, located in Fresno, California. A single trial of self-selected gait velocity was measured upon admission and discharge using the 10-meter walk test. A paired t-test was used to analyze differences between admission and discharge of each participant and a Spearman's Rho correlation analysis was conducted to determine a possible association between length of stay and discharge self-selected gait velocity.Results: Participants had a mean self-selected gait velocity change of 0.18 m/s (SD 0.19) from admission to discharge. The average length of stay was 12.43 days (SD 3.44), and all participants were discharged home. This study revealed a statistically significant difference in self-selected gait velocity between admissionand discharge (t = 3.470), 95% CI [0.07, 0.29], p = 0.004. Spearman's rho correlation analysis revealed an inverse, low to fair correlation between length of stay and discharge self-selected gait velocity (r = -0.37, p = 0.19) but no statistical significance was found.Conclusion: While a statistically significant change in gait velocity was noted, no significance was found between length of stay and discharge self-selected gait velocity. Future studies should explore intervention strategies and predictors of length of stay to help facilitate the management of patient outcomes within this setting.
