Background: The utilization of hospice services is growing substantially in the United States due to the increased prevalence of chronic conditions and the increasing population. Hospice care focuses on maintaining quality of life over quantity of life. With the direct focus on quality of life comes hurdles that providers must overcome, such as pain management. Unmanaged pain at the end of life is hospice patients' greatest fear and the most common concern. Research shows that through continued education, adequate communication, and consistent documentation, hospice providers and nurses successfully assess, manage, and document pain at the end of life. Methods: A quality improvement (QI) project was developed to improve pain experienced at the end of life for hospice patients and increase providers' and nurses' confidence in managing pain. The Pain Assessment and Documentation Tool (PADT) and an education seminar were implemented at a community-based hospice to support this goal. The nurses and providers completed surveys regarding confidence in pain assessment and documentation before the education, after the education, and during the implementation of the PADT. The tool was implemented in practice for four weeks. Retrospective chart reviews were completed to assess patient-reported pain four weeks before the education and for the four weeks during the implementation of the PADT. Results: Nurse and provider confidence in managing patient pain increased from 97% to 100% of participants following the interventions. Confidence in the documentation of pain management increased from 65% to 100% of participants post-implementation. Hospice providers found the nursing report more detailed and increased requests for long-acting pain medications. Patients "Most Recent Present Intensity of Pain" decreased by .482 points on a pain scale of 0 to 10, 0 being no pain and 10 being the worst pain. Conclusion: Results support the need for continued provider and nurse education regarding pain assessment and management in the hospice setting. The PADT was found helpful in practice and should be tailored to meet the individual organization's needs.
