Reducing unnecessary emergency department (ED) transfers and hospital admissions is essential for improving care in assisted living facilities, especially given the growing aging population and increasing financial burden on the healthcare system. This study evaluates the impact of onsite physician practices on engagement rates between facility staff and physician practices during triage and their effect on ED transfers and hospital admissions. A quasi-experimental difference-in-differences design compared assisted living facilities with onsite physician practices (treatment group) to those using external primary care providers (control group). The primary outcomes measured were ED transfer rates, hospital admissions, and engagement levels during triage. The study found that onsite physician practices significantly increased engagement between facility staff and physicians during triage. However, this increased engagement did not lead to a significant reduction in ED transfers or hospital admissions. The findings suggest that while onsite physician practices enhance communication and decision-making during triage, they alone are insufficient to reduce unnecessary ED presentations and hospital admissions. One reason for the lack of significant outcomes is the restrictive regulations and reimbursement structures outside Medicare Advantage plans, which limit essential services like palliative and home health care. Future interventions should address clinical variables prompting acute care services, communication barriers, staff fears regarding resident acuity, and access to community services. This study underscores the need to re-evaluate current policies and reimbursement models to integrate onsite physician practices and comprehensive care services in assisted living facilities. Addressing these issues is crucial to managing the increasing demand for post-acute care and reducing the financial burden on the healthcare system, ultimately improving the wellbeing of the elderly population.
