This direct practice improvement project investigated the effect of transitional care management provided by case managers to adult Medicare eligible individuals over the age of 18 years, on hospital discharge to home, in decreasing readmission rates within the initial 30 days' post discharge. Previous studies showed transitional care management and the use of telephonic case management decreased readmission rates. The literature identified the gap in transitional care resulting in higher readmission rates. This project advances the current literature to support the use of transitional care management by case managers to reduce readmission rates. A quantitative methodology and a causal comparative research design were utilized to retrospectively review the participants clinical record. The sample derived from the population at a large national health plan with a total of 64 participants equally divided into two groups, those who received the intervention and those who did not. Both descriptive and inferential statistics were utilized to analyze the data. Theoretically, the integrated approach of the adult learning theory and Pender's Health Promotion Model created an environment where education did occur. The results found participants in Group 1 and group 2, were equally likely to be readmitted within 30 days of discharge date, chi2 (1, N = 64) = 1.896, p = .168.
