As peri-operative nurse leaders continue to face the multifaceted challenges in their administrative roles, they may experience personal challenges with self-care practices, self-regulation, and stress management.

The study is based on current literature and incorporates self-determination theory (SDT) as the theoretical framework. An exploration of the unique narratives from eight current peri-operative nurse leaders provided insights into specific elements of their role and the person-environment dynamics in the peri-operative setting that impact the leaders' well-being.

The SDT principles of autonomy, competency, and relatedness served as the framework to distill common themes from their narratives about their work experiences and the peri-operative environment that influenced their self-care practices. Consistent with narrative method, their synthesized stories highlighted the significance of autonomy and decision-making while working in an organizational culture that supports and models self-care and actively engaged administrators who support a healthy work environment free of incivility and bullying.

The participants' insights illuminated the disconnect between the decades of research that outline the longstanding trends of self-care deficits in nurses and the growing and unresolved recruitment and retention issues in this critical nursing specialty. The narrative themes provide a deeper understanding of the seemingly uncorrectable self-care challenges faced by today's peri-operative nurse leaders.
