BACKGROUND: According to the United Nations World Population on Ageing the world's population is rapidly aging. As the population ages, there are increased concerns for the nutritional status of the elderly. A large body of evidence indicates that awareness of malnutrition and nutrition-related problems in the elderly population living at home is generally low.

PURPOSE: The purpose of this study was to identify the risk factors for malnutrition in the elderly population living in the community.

METHODS: This quantitative study utilized data from the National Health and Nutrition Examination Survey (NHANES) 2011 -2012. Socio-economic, demographic, dietary intake, biochemical lab values, cognition and physical functioning data were collected from 1,472 participants. The data were analyzed using SUDAAN software. The statistical procedures used included chi-square tests and t- tests.

RESULTS: Approximately 34.5% of the elderly population were malnourished. The prevalence of malnutrition increased with age, and was highest in African Americans and those who were widowed. The mean dietary intake of protein was significantly lower in malnourished subjects. Malnourished subjects were significantly more likely to experience difficulties in thinking and remembering, and have lower levels of physical functioning. Except for vitamin B12, malnourished subjects had significantly lower levels of hemoglobin, hematocrit, red blood cell count, albumin, total protein, iron, calcium, blood urea nitrogen, creatinine, and red blood cell folate.

CONCLUSIONS: Older age, marital status (widowed), lower education levels, lower incomes, cognitive and functional impairment, poor intakes of dietary protein, as well as the presence of medical conditions such as anemia, stroke and cancer and food insecurity were major risk factors associated with malnutrition in community-dwelling elderly.

RECOMMENDATION: More large scale studies to identify modifiable risk factors of malnutrition in community dwelling elderly in the United States that will result in the implementation of policies and services to minimize risk are recommended.
