Design: This study was conducted in two phases. Phase 1 used the Delphi Approach. Phase 2 involved a qualitative analysis of observations and interviews guided by The Information Chaos Model.

Settings and participants: For Phase 1, 14 LTC dispensing pharmacists were recruited through the Pharmacy Society of Wisconsin Fast Facts Listserv to participate in the Delphi Approach. For Phase 2, seven LTC dispensing pharmacists in five long-term care pharmacies were recruited to collect data on ten nursing home admissions regarding the types and quality of information that is used to process orders and/or perform medication reconciliation.

Measurements: For Phase 1, 80% consensus on the types of information needed to perform medication reconciliation was measured around three points on a nine point Likert scale. For Phase 2, types and quality of information received and used was analyzed using thematic analysis and coded using the constructs from the Information Chaos Model. Information obtained in the follow up interviews was categorized as to what constitutes a good or poor quality admission as it pertains to the quality of information the pharmacists receive.

Results: Phase 1: At the completion of the fifth questionnaire, 22 of 34 types of information met consensus regarding the importance of the need to have to perform medication reconciliation.

Phase 2: The pharmacists encountered many situations in which they received too much information, incomplete information, information that they received at different times or information that conflicted. Pharmacists articulated that receiving information that was clear, concise and accurate and presented in an organized format contributed to what they perceived as a good quality admission.

Conclusions: The findings from this study can be the foundation for further research on the appropriate role of LTC dispensing pharmacists in nursing home transitions of care.
