Parent-Child Interaction Therapy (PCIT) is a parent training program that has empirical support in the treatment of young children with high levels of behaviors associated with externalizing disorders. While PCIT is very effective for reducing disruptive behaviors in children, understanding the effects of the treatment on parenting stress is somewhat limited. Unfortunately, a high level of parenting stress in PCIT is associated with a variety of undesirable outcomes including parents dropping out of treatment, slow progress, or failing to progress altogether.

The current study evaluated the relationship between PCIT and parenting stress through both phases of PCIT. Five parents with clinically significant levels of parenting stress began PCIT treatment after an established baseline and completed the Parent Stress Index 4th edition (PSI-4-SF) and the Eyberg Child Behavior Inventory (ECBI) weekly.

Results were analyzed using visual inspection and the Standard Mean difference of all intervention points. Additional analysis was conducted to explore the relationship between ECBI scores and parenting stress, PRIDE Skill use and parenting stress, the length of treatment and parenting stress, and the PSI-4-SF subscales and PCIT.

Of the five participants in the study, four participants had a visually notable and statistical difference in parenting scores by the end of PCIT, as hypothesized. Only one participant did not show a notable difference in scores by the end of CDI, and four participants had a steeper decline in parenting stress during the PDI phase of treatment than during the CDI phase of treatment, as predicted. Furthermore, by the end of treatment three out of five participants' parenting stress scores were within normal limits although their parenting stress scores had been within the clinically significant range at the end of CDI. Of note, in spite of the changes in parenting stress, additional analyses for parenting stress subscales showed that by the end of treatment four out of five participants still reported clinically significant child behavioral adjustment issues and behavioral symptoms and three out of five parents still reported clinically significant issues with their parent-child bond.

Overall, the results suggest that PCIT is a very effective treatment in reducing parenting stress and oppositional behavior in children of parents with clinically significant levels of parenting stress but may not be enough to reduce parenting stress to within normal limits for all parents.
