In the United States approximately 500,000 children annually enter the foster care system due to neglect, sexual, physical, or emotional abuse. According to The Adoption and Foster Care Analysis and Reporting System's (AFCARS) annual preliminary report children are removed from their homes for various reasons such as, neglect, parental substance use and abuse, physical abuse, behavioral problems, inadequate housing, sexual abuse, relinquished parental rights and or the death of a parent. Additionally, approximately 250,000 (50%) of these children are placed in out-of-home settings (i.e. foster homes, group homes, residential treatment centers) and stay in the foster care system for approximately 12 to 17 months. During this time period, foster youth may be re-traumatized; experience placement instability, which is defined by "three or more out-of-home placements during a 12-month period"; because of the youths' age, race, ethnic identity, gender, mental health needs or behavioral issues (The Massachusetts Citizen for Children, 2011). This clinical dissertation explores the history of Children Protective Services, group home placements and residential treatment centers, identifies common characteristics of foster youth, examines placement instability, and discusses Trauma-Focused Cognitive Behavioral Therapy as an intervention for reducing placement disruption. This dissertation provides a critical review of the literature, integrates clinical information gathered from expert and emerging field consultants, who have worked closely with the Department of Children and Family Services, the Department of Mental Health, Department of Corrections and non-profit agencies that service at risk foster youth, and discusses quantitative and qualitative data gathered from the final presentation audience members. This dissertation also includes the final presentation slides and handouts, presentation evaluation and feedback data collected from the audience members, and consent forms. Additionally, this dissertation discusses and provides a critique of the literature, explores the author's creative process, and identifies areas for future research and professional growth.
