Injury factors and physical traits of overhead athletes with shoulder pathologies, such as range of motion (ROM), strength and upper quarter dynamic balance, have been investigated by previous studies. However, the comparisons between populations with shoulder injuries and those without any shoulder problems were not comprehensive. Therefore, the purpose of this study was to compare shoulder characteristics, including shoulder ROM, strength, and upper quarter dynamic balance, between shoulder injury population and healthy population in collegiate overhead athletes. A total 58 NCAA division 1 overhead athletes (baseball, softball, volleyball, and tennis), 25 of the injury group and 33 of the healthy group, participated in this study. Participants in the injury group had been diagnosed with any shoulder injuries, and those in healthy group did not have any medical history of shoulder injuries. Participants performed shoulder internal rotation (IR), external rotation (ER), and horizontal-adduction (HAD) active ROM measures, IR and ER strength measures, and Upper Quarter Y Balance Test (UQYBT). The results of each measurement were compared to analyze the intergroup differences using one-way ANOVA (alpha≤.05). The injury group showed a lower UQYBT mean score in the superolateral direction ( p=.033). However, there were no statistically significant intergroup differences in shoulder IR ROM (p=.541), ER ROM ( p=.921), HAD ROM (p=.946), IR strength ( p=.280), ER strength (p=.238), ER/IR strength ratio (p=.377), medial direction of UQYBT (p=.716), and inferolateral direction of UQYBT (p=.493). Previous shoulder injuries in the injury group had been completely treated, and all participants did not have any ongoing symptoms on their dominant shoulders. However, significant IR ROM deficits existed in both groups, along with the lower UQYBT mean scores (superolateral directions) in the injury group. Therefore, well-planned ROM, strength, and upper quarter dynamic stability exercise programs for injury prevention and rehabilitation may be warranted for all overhead athletes.
