Nursing is a physically and mentally exhausting profession that may inadvertently cause various psychological consequences while caring for others. The purpose of this study was to gain an understanding of baccalaureate nursing student experiences related to their decision to go into nursing, the implications for their reactions to future patient care, and the identification of trauma informed care principles from these themes in a baccalaureate nursing elective course. Students were asked to reflect on these events by describing the event, their reactions, personal context, and implications for future practice as nurses.Using qualitative content analysis, themes from 14 self-awareness course assignments were described and interpreted. Situations fell into three categories: illness to self, illness to others, and negative situations such as a negative comment or toxic environment as some of the students had previous experience in healthcare outside of being a nurse. Emotions to these events were predominantly negative, with a total of 28 codes for negative emotions such as scared, nervous, sad, and unhappy. Only seven codes mentioned positive emotions, most commonly relief, and only two identified mixed emotions (both a positive and negative emotion such as anger and relief). Ten of the fourteen people identified physical reactions to the events such as sickness, fatigue, freeze/shock, or flight/flight.Three themes arose across the data related to beliefs: uncertainty, struggling/suffering of others, and support. Uncertainty was broken down into three categories, "from environment," "from others," and "from self" and was coded 30 times. Struggling/suffering of others was coded ten times. Support was coded five times. Beliefs related to what students experienced that led them into nursing involved significant reflection on the events, with self-awareness to their reactions that could fuel their future nursing practice.When asked to describe how these experiences could be used in their future nursing practice after graduation, 81 codes arose that were categorized into four themes: social awareness, cognitive awareness, emotional awareness, and intuitive awareness. Social awareness had 31 lines of code, cognitive awareness had 28 lines of code, emotional awareness had 16 lines of code, and intuitive awareness had six lines of code. Social awareness was further broken down into four categories: "to patients," "to patients' friends/family," "from others," and "to peers." Awareness is a key element of transformative learning theory. Social, cognitive, emotional, and intuitive awareness represent skills that are not commonly covered in baccalaureate nursing programs. Reflective learning is an important part of that process. Awareness leads to change, and it is this change in behaviors, thoughts, and even emotions that result in transformation. It is posited that this transformation could be protective for nurses entering such a psychologically demanding and potentially traumatic profession.Themes were then compared to trauma-informed care (TIC) principles. TIC proports that potentially traumatic experiences can have several long-term consequences. Most events described could be considered potentially traumatic and participants described fear as the most common response. All six TIC principles: safety, transparency, support, collaboration, empowerment, and inclusivity were mentioned in the context of the reflective assignments. The focus on this type of reflection is to learn from events, as opposed to focusing on what went wrong in events, which was evident in the link between themes and TIC principles.The potential for psychological trauma that can ensue from the profession of nursing and the subsequent sequalae of physical and mental health, disordered substance use, and a host of social determinants of health influences on individuals and the nursing profession as a community are concerning. The nursing vocational crisis and unsustainable rate at which nurses are leaving the profession impacts the health of our nation. Academic nursing has an obligation to prepare nursing students both academically and in terms of ongoing professional development, which should include personal development, so they can enter the profession being able to acknowledge their limitations and resources and respond to a host of situations with skills to stay healthy and embrace the profession and regularly renew their professional purpose.
