Skin cancer affects millions of individuals in the United States, with nearly 9,500 new diagnoses daily. Often when patients have a lesion of concern, they present to their primary care provider (PCP) for evaluation. PCPs usually perform skin assessments with naked-eye examinations where the provider uses only their eyes to analyze the skin. However, this type of exam can lead to inconsistent results due to varying assessment techniques and knowledge of skin lesions. Skin examinations can also be performed with dermoscopy, a handheld tool that uses light and magnification to improve the evaluation of skin lesions. Dermoscopy training programs have been shown to increase PCP confidence, diagnostic accuracy, and triaging of skin lesions. For this reason, an online module on dermoscopy and skin lesion triaging was created and disseminated to the PCPs at Family HealthCare (FHC), a clinic that offers affordable health care services to uninsured patients and people of color. To increase the accessibility of the module for PCPs outside of FHC, the module was distributed to online platforms, including the North Dakota Nurse Practitioner Association (NDNPA) webpage and a variety of nurse practitioner Facebook groups. This practice improvement project focused on improving skin cancer screenings in the primary care setting by enhancing skin lesion triaging and lesion identification with dermoscopy. Additionally, the project aimed to reduce the disparities found in uninsured and underserved patient populations related to skin cancer. Pre-and post-implementation surveys and module quiz question results were utilized to assess the objectives of increasing provider knowledge, confidence, and application of dermoscopy. The providers at FHC were also given dermascopes to practice with over a three-month implementation period. Results from the project support findings from the literature, with perceived improvement in knowledge and confidence with skin lesion triaging and identification following dermoscopy training and clinical practice with dermascopes. Given the prevalence of skin cancer in the United States, PCPs are essential for completing and promoting skin examinations. With the enhanced knowledge of skin lesion triaging with dermoscopy, PCPs can improve patient outcomes through early detection and by reducing barriers to care.
