Depression is more closely associated with a lack of daily living activities and neurochemical changes in the brain. The most common symptom of Parkinson's disease is depression, which results in significant disability and an increased burden on caregivers. Therefore, treatment for depression becomes essential for people with Parkinson's disease. Psychopharmacology is generally used to treat depression in Parkinson's disease; however, it has many adverse effects, including worsening orthostatic hypotension, constipation, and cognitive impairment. Evidence shows that complementary therapies can be beneficial for treating depression alone or in conjunction with medications for people with depression. Complementary therapies such as cognitive behavioral therapy (CBT) can effectively promote constructive thinking and regular participation in everyday activities, which may help reduce depression. There is a need to be aware of the current literature findings regarding CBT's effectiveness when treating depression in people with Parkinson's disease, and we conducted a narrative review. According to the findings of the narrative review, CBT interventions helped treat depression associated with Parkinson's disease. However, our narrative reviews lacked specific details describing and summarizing the relationships between variables. Hence, we conducted a meta-analysis of the effects of CBT on depression in people with Parkinson's disease. We coded data from prior studies, including source, method, intervention, and participant characteristics. We analyzed the data using the random-effects model and used Hedge's g to estimate the effect size. We examined publication bias and heterogeneity. We conducted a moderator analysis to examine the effects of moderator variables. Our results revealed that CBT significantly decreased depression among people with Parkinson's disease. Through moderator analyses, we identified several moderators that contributed to the effect sizes of CBT on depressive symptoms, including the source of data (e.g., funding); method characteristics (e.g., intention-to-treat, attrition, number of quality indicators); participants' characteristics (e.g., age, Parkinson's disease duration); and intervention characteristics (e.g., days/weeks across the CBT intervention). The results of this meta-analysis provide clinicians and health providers with evidence that supports the use of CBT as an adjunct or complementary treatment for people with Parkinson's disease.
