Eating disorders (ED) are complex illnesses that often have significant physical, psychological, and financial costs to adolescents and young adults and families. This dissertation focuses on ecological, treatment, and financial factors associated with outcomes related to ED - examining ED across the course of illness continuum that ranges from emergence of cognitions and behaviors, visible symptoms, screening and identification, treatment, and disease outcomes. It is not known how the recent COVID-19 Pandemic (COVID-19) impacted the entire course of illness continuum specifically among young people with restrictive ED. Additionally, a key underlying aspect of EDs are the cognitions that drive the associated behaviors. A gap exists in the literature around the association between ED symptomology, i.e., behaviors and cognitions, and psychiatric medication use in adolescents and young adults with restrictive EDs over time. Finally, while research has shown that there are many outcomes associated with ED, a gap in the literature still exists around examining the secondary outcomes of out-of-pocket financial costs that parents and families report.This dissertation includes three studies. Aim 1 used a scoping review to look at how COVID-19 impacted adolescents and young adults with restrictive ED. The scoping review found that the pandemic influenced how ED treatment was received, including an increased use of telemedicine as a treatment modality, an increase in medical hospitalizations related to eating disorders, changes in ED-related symptomology, and poorer overall behavioral and mental health.Aim 2 is a longitudinal analysis looking at the association between psychiatric medication use and ED symptoms among 79 young people with anorexia nervosa or atypical anorexia nervosa over 18 months. In the longitudinal analysis, we found that youth who reported psychiatric medication use had an average decrease of 0.07 fewer affected days/month over time, however this was not significant. Furthermore, no significant difference was found in the average number of affected days/month decrease among youth who reported psychiatric medication use relative to youth who did not report psychiatric medication use over time.Aim 3 was a content analysis of parental reported out of pocket financial costs associated with their child's ED treatment. Results from the summative content analysis revealed three main categories that highlight how the eating disorder impacts the entire family: Treatment-related Issues, Increased Living Expenses, and Reduction in Income. Our findings reinforce that the risk for, course of, and secondary outcomes related to ED are impacted by multiple factors. In conclusion, this dissertation fills gaps in the literature by exploring key ecological, treatment, and financial factors associated with outcomes related to ED among youth and their families.
