Depression ranks among the most prevalent mental health disorders, as it impacts the quality of life of individuals, as well as increases in expenditures due to treatment (The United States Preventive Services Task Force (USPSTF), 2023). Therefore, recommended practice guidelines indicate the need for depression screening for patients. According to the literature, depression screening is best completed during primary care visits. However, this is not a standardized process in primary care. Therefore, this quality improvement project aims to increase depression screening and referral rates at the primary care clinic using the Patient Health Questionnaire (PHQ-9) paper form to screen adult patients during their scheduled visits over six weeks.
