Despite efforts to decrease Clostridioides difficile infection (CDI) with antibiotic stewardship and improved sanitation, it continues to be a major public health problem. Prior studies have linked dietary components to modulating CDI, which indicates that diet could be a potential tool in CDI prevention. In a previous study conducted by our lab, we found that mice on a high-fat/low-fiber "Western" diet had increased mortality compared to mice on a low-fat/low-fiber and chow (control) diet, demonstrating that dietary fat plays a role in increasing disease severity with antibiotic-associated CDI. Dietary fiber has previously been implicated in promoting clearance of C. difficile through the production of the beneficial metabolite butyrate, but its role has not been investigated in the context of a high-fat diet. Here, we show that mice fed both highfat and low-fat purified low-fiber diets had high mortality in an antibiotic-induced C. difficile murine model, and that supplementation with a cocktail of 4 soluble/insoluble fibers improved survival. Mice on high fiber diets had higher cecal and colonic inflammation, a healthy gut hypoxia gradient, and a larger composition of CDI-inhibitory bile acids compared to mice on low fiber diets. Mice on high fiber diets also showed increased gut microbiome resiliency to antibiotic disturbance, recovery of diversity after release of antibiotic pressure, and control of potentially pathogenic facultative anaerobes. These findings indicate that diets high in fiber can aid in lessening C. difficile disease severity and mortality and could be valuable to employ in high-risk populations.
