Background: The responsibility of childhood obesity prevention is complex and involves multiple stakeholders including early care and education (ECE) settings as well as parents, communities, government sectors, organizations providing education and training to ECE providers, and child care licensing bodies. Identification of stakeholders and the development of a State Action Plan will provide a network of invested contributors working towards a shared goal of obesity prevention in ECE settings in a more cooperative and comprehensive manner.

Objectives: The purpose of this study is to 1) identify key stakeholders and existing collaborations pertaining to obesity prevention in the ECE environment, 2) assess current obesity prevention efforts implemented by stakeholders and 3) develop a State Action Plan to increase communication and connection between stakeholders to improve quality and efficiency of state-level efforts.

Methods: This mixed method, descriptive research study included ECE stakeholders (n=127) that are involved with obesity prevention efforts in Oklahoma. To address aim 1, a quantitative assessment of existing networks and collaborators were identified and then the strength and clustering of those relationships were measured. A quantitative, stakeholder network analysis (SNA) was created to map and measure the connections between invested parties and to identify key stakeholders using UCINET 6 for Windows, version 6.620 and NetDraw, version 2.159 network visualization software. To address aim 2, a qualitative assessment of existing obesity prevention efforts was conducted to derive information of whom and what efforts regarding obesity prevention were ongoing. Qualitative reviews of publically available materials and interviews with the stakeholders were also conducted. Interviews were guided by a modified partnership assessment worksheet (PAW) designed for state and local leaders. To address aim 3, a second stakeholder meeting was convened to develop a State Action Plan.

Results: From the quantitative assessment, the study identified 25 out of 127 individuals as key stakeholders that connected various people and agencies. In addition, the SNA has demonstrated that 6 clusters of various agencies exist, as well as 3 agencies that are isolated. In regards to the qualitative assessments, 70 stakeholders have reported on a wide-range of ECE obesity prevention efforts which include gardening and physical activity curricula, six different health coalitions, resources that offer technical assistance and assessment tools for ECE facilities, grants to provide funding for obesity prevention projects for children, as well as various health department wellness programs. The details of each of these efforts were collected and disseminated to stakeholders to make it easier for others to know what was occurring within Oklahoma. Further, a detailed State Action Plan was constructed to inform the stakeholders of the next steps in regards to obesity prevention for early childhood. Some examples of areas to address include better assistance with ECE provider training, greater access to fruit and vegetables/gardening resources, increased family outreach, further policy updates, etc.

Discussion/Conclusions: Creating a more interconnected network and utilization of key ECE stakeholders has led to increased communication and potential for more collaboration within ECE regarding obesity prevention. This also helped guide a more direct and unified State Action Plan with identified areas of improvement. Addressing the gaps within existing ECE prevention efforts may help to combat childhood obesity in a more comprehensive manner. Overall, the State Action Plan has allowed ECE stakeholders to collectively work together, and in turn prevent childhood obesity in the state of Oklahoma.
