Lack of practical knowledge and accordance to the low-sodium diet (< 2,000 mg) in heart failure (HF) patients is associated with worsening symptoms, a decrease in quality of life, and hospital re-admittance rate. Therefore, 88 HF participants {mean age +/- SD, 54 +/- 14 years) from an outpatient HF clinic were surveyed about practical sodium knowledge using a knowledge questionnaire (low knowledge 0-3; high knowledge 4-10) and accordance to a low-sodium diet using a sodium screener. The majority of participants were non-Hispanic Black (69.3%) and male (56.8 %). Median (IQR) knowledge was 7 (3, 10), 71.6% had high knowledge, and only 33% knew the sodium guidelines (2,0003,000 mg/day). Participants with > 12 years of education sorted more food containers correctly (P = 0.013) and could more accurately read the food label (P = 0.001) than those with less education. Despite high knowledge, 64.8% of HF participants were not accordant to the low-sodium diet (2,602 +/- 1,412 mg sodium intake). Barriers to accordance included low-sodium foods not tasting good (35.2%), difficulties eating outside the home (28.4%), and low-sodium foods being expensive (11.4%). High sodium knowledge and low accordance provides evidence for registered dietitians to incorporate behavioral strategies into HF nutrition intervention.
